Hi,
Yes it means going up or down on the topa in small amounts. I took this from another post which is based on the book
week 1 - 1 x 25mg in afternoon
week 2 - 2 x 25mg in afternoon
week 3 - 1 x 25mg in morning and 2 x 25mg in afternoon
week 4 - 2 x 25mg in morning and 2 x 25mg in afternoon

By week 8 it has you up to 300mg but I think you just have to follow the schedule and see what you can tolerate. Aa lot of people can only handle 150mg in a day.
